3/8/2024 EnchantaVaughn Approved Behavior: Upon arrival, JaCarei was confronted by his mom for talking back when she asked him to clean up and get ready for his session. JaCarei was sent to have one on one with his counselor immediately following this altercation. JaCarei tried to lie about talking back and said his mom was only mad because he was making a lot of noise in the house. JaCarei finds himself off task and interrupting QMHP, indicating he wasnât aware of his misbehaviors and misconduct at which mother reports behaviors last up to 3-4 minutes in duration. Behavior: After reviewing ISP, JaCarei shows difficulty in following home rules often choosing to act out on purpose. Reinforcement was provided to JaCarei during psycho educational. Mom also reports that JaCarei refuses to listen to her or do what sheâs asked, and it has become toxic in the home. JaCarei displays outbursts when confronted about negative behaviors he displays, as if heâs not responsible for these concerns. It is evident, JaCarei lacks respect for authority figures and cannot admit when heâs wrong at which he will lash out on other with behaviors lasting up to 10 minutes in duration. Intervention: QMHP observed JaCarei in session in efforts to determine whether he required assistance. QMHP engaged JaCarei in a conversation regarding his recent behaviors, progressions and setbacks. QMHP monitored progress in demonstrating individualized pro-social skills (e.g. anger management, problem-solving, conflict resolution, appropriate verbalization of feelings, and cognitive-behavioral programming). QMHP talked with JaCarei about the importance of adhering to rules and expectations at home and in the community. Intervention: QMHP held consultations with JaCarei and mom about effective intervention strategies that build on the JaCarei's strengths and compensate for his negative behaviors. QMHP explored JaCareiâs responses and responded accordingly. QMHP assisted JaCarei with problem-solving skills. QMHP encouraged JaCareiâs mom to provide frequent positive reinforcement in helping him complete his objectives daily. QMHP encouraged JaCarei to practice relaxation and coping techniques in social/virtual environment as often as possible. Response: JaCarei engaged in self-reflection, discussing good and bad parts of his day. JaCarei communicated that he wants to work independently. JaCarei modeled different ways to walk away from a conflict when frustrated. JaCarei demonstrated that he was able to process productive expressions of anger. JaCarei exhibited how to verbalize her feelings without yelling and being aggressive, which allowed others to show empathy and support regarding his emotions. Response: JaCarei was observed in session utilizing and learning new interventions and strategies with his family. JaCarei realized his mom was trying to help him by pushing him to try first and then get more help from counseling. JaCarei appeared to be learning from his mistakes and correcting them while QMHP went over his treatment plan with his mom and discussed information that provoked behaviors from today. JaCarei learned to avoid negative and defiant behavior and comply to rules.
